Employee Stock Option Plan
The PowerVision Corporation Employee
Stock Option Plan recognizes outstanding employees by awarding stock
options. These options acknowledge the contributions of employees
to the continued growth, development, and financial success of the
company.
Life Insurance
Exempt employees are covered by the Group Life
coverage of Continental Assurance Company. PowerVision
pays the premiums.
Short Term Disability/Long Term Disability
and Accidental Death and Dismemberment Insurance (STD/LTD and AD&D)
Exempt employees are automatically enrolled with
Continental Assurance Company for LTD, STD, and AD&D.
PowerVision pays the premiums back to you for the provided
coverage.
Flexible Spending Accounts (FSAs)
Pre-tax money can be set aside to help cover non-reimbursed
medical and dental fees such as co-pays, deductibles, or other out-of-pocket
expenses and daycare or childcare costs. Employees process claims
for reimbursement from their accounts.
401(k) Retirement Fund
Employees are eligible to start contributing in
the first full month of employment. Contributions can be as
high as 15% of your salary.

Eligibility
Full time exempt employees are eligible for single
or dependent group coverage immediately upon their first day of
employment with PowerVision.
Medical Coverage
United Health Care provides coverage for PowerVision.
The employee contributes only when Dependent coverage is selected.
PowerVision offers the additional benefit of enabling employee
contributions to be made on a pre-tax basis.
PowerVision covers the cost for the employee.
Group coverage for dependents is available through a monthly payroll
deduction. Employee contributions are as follows:

Dental Coverage
Metropolitan Life Insurance Company provides dental
benefits for PowerVision. Plan cost is covered by PowerVision.
COBRA Coverage
COBRA continuance, for both medical
and dental insurance, is available in the event of termination of
employment.
Vacation
- 15 days for the first 3 years
- 16 days during the 4th year
- 17 days during the 5th year
- 18 days during the 6th year
- 19 days during the 7th year
- 20 days during and beyond the 8th year

Employees can carry forward up to 120 hours, or 15 days, of
paid time off for one calendar year. Hours above 120 hours
cannot be carried over and must be used or lost.
Sick Leave
- 3 days sick leave each year
Employees can carry forward up to 40 hours, or
5 days, of sick leave each calendar year. Hours above 40 hours will
be lost.

Interest
Tuition Reimbursement
PowerVision will reimburse the cost of
one course per semester for a work-related subject for employees
who have completed one year of full-time service. Reimbursement
is for pre-approved courses and is made after successful completion
of the course.
Internet Account
PowerVision pays for one dial-up Internet
account for employees. This is in addition to the corporate
access and e-mail account assigned to each employee. Reimbursement
claims are filed every six months.

One-time certification bonuses for professional
certifications and designations are awarded for the following certifications:
